Massage Guns: What Percussive Therapy Does, and What It Does Not

A massage gun is a handheld device that delivers rapid bursts of pressure and vibration into muscle and connective tissue, a method usually called percussive therapy or percussion massage. It has become a common sight in gyms and physiotherapy clinics, marketed for warm-up, mobility, and recovery. This guide is general and educational, and it stays brand-neutral: it describes what the category of device does and does not do according to the research, not which model to buy.

The honest summary is narrower than the marketing. A short percussion session produces a real, measurable increase in joint range of motion without weakening the muscle, which is a genuine advantage for warm-up mobility. The case for using one as a recovery tool is more tentative: the evidence is emerging, mixed, and based on small studies. And several popular claims, that a massage gun flushes out lactic acid, breaks up scar tissue, or substitutes for medical care, are not supported by the evidence. The essentials at a glance

What percussive therapy actually does

The best-supported effect of a massage gun is an immediate increase in range of motion. In a controlled study by Konrad and colleagues in 2020, sixteen healthy men received a single 5-minute percussion treatment of the calf muscles with a handheld device on one test day and sat quietly on another. Maximum ankle dorsiflexion range of motion increased by 5.4 degrees, about 18.4 percent, after the percussion treatment, with no change in the control condition. Just as importantly, maximal voluntary contraction torque, a measure of muscle strength, did not change after the treatment. The authors concluded that range of motion can be increased by a handheld percussive massage without having an effect on muscle strength. A 2024 study co-authored by Konrad reproduced the pattern: percussion increased dorsiflexion range of motion and stretch tolerance and reduced passive stiffness, again without reducing strength.

That combination, more mobility with no loss of force, is what makes percussion useful before training. Prolonged static stretching can temporarily reduce strength and power output, which is why it has fallen out of favor as a final pre-lift warm-up. A short percussion session offers a way to gain joint range of motion without that trade-off. The mechanism appears to be largely neuromuscular rather than mechanical: the gain tracks with improved stretch tolerance and reduced passive stiffness, not with any physical reshaping of the tissue. The practical reading is that percussion is a reasonable mobility primer, used briefly and at a comfortable intensity, as part of preparing to move.

The recovery and soreness evidence is still emerging

The claim that gets people to buy a massage gun is recovery: the idea that using one after hard training reduces next-day soreness and speeds a return to performance. Here the evidence is genuinely promising but far from settled. A 2025 randomized controlled trial published in Frontiers in Public Health divided thirty physically active young men into three groups after a heavy squat session designed to induce delayed-onset muscle soreness: static stretching, a short 25-minute percussion session, or a long 40-minute percussion session, with measures taken at 24 and 48 hours. Percussion therapy was more effective than static stretching for soreness recovery, and the two 40-minute sessions provided greater benefit than the two 25-minute sessions. That points to a real recovery effect, and to a dose dependence in which longer sessions helped more.

The important caveats sit alongside that result. It is a single small trial of thirty young men in one muscle group, and its own authors note that there is limited evidence supporting percussion against traditional methods. A broader picture comes from a 2023 systematic review by Ferreira and colleagues, which screened 281 records and could include only eleven studies, ten of moderate and one of high risk of bias. The review concluded that massage guns can help with short-term range of motion, flexibility, and recovery-related outcomes, while flagging the moderate quality and small number of studies as a real limitation. In other words, the recovery case is built on a young and thin evidence base. A massage gun may help soreness, much like the comparable tool covered in the guide on foam-rolling, but neither should be oversold as a settled recovery method, and neither is a substitute for the basics of sleep, nutrition, and sensible training load.

What the evidence does not support

Several familiar selling points do not hold up. The most common is that a massage gun flushes out lactic acid or toxins. The 2023 systematic review found no difference in lactate concentration after percussion use, so there is no evidence for a lactate-clearing effect, and lactate is in any case cleared rapidly by the body on its own rather than lingering as a cause of soreness. A second claim is that percussion breaks up scar tissue or fascia. There is no good evidence that a handheld device mechanically reshapes connective tissue; the measured benefits track with neuromuscular changes such as improved stretch tolerance, and breaking up scar tissue remains a hypothesis rather than a demonstrated effect. The range-of-motion gain, while real, is also short-lived, lasting minutes rather than producing a lasting change in flexibility.

There is also a question of when not to reach for a massage gun. The same review noted that percussion either did not improve, or in some cases reduced, performance in strength, balance, acceleration, agility, and explosive activities, so it is not a tool to apply immediately before a maximal effort. And percussion is not a treatment for an injury. A device that delivers force into tissue can make a strain, a fresh injury, or an inflamed area worse, and it does not replace assessment and care from a qualified professional. Where should you not use a massage gun?
Keep a massage gun away from bones, joints and implanted hardware, nerves and blood vessels, and sensitive areas such as the face, head, the front of the neck, chest, and spine. Avoid bruises, open wounds, skin infections, and any acute or inflamed injury, and do not use it aggressively on one spot for a long time. People with a clotting disorder or deep-vein thrombosis, those on blood thinners, people with diabetes or reduced sensation, and those who are pregnant or have a pacemaker should check with a clinician first.
Can a massage gun cause damage?
Used appropriately and at a comfortable intensity, a massage gun is generally considered low-risk for most healthy people. Harm tends to come from misuse: aggressive or prolonged use on one area can cause muscle and tissue damage, and applying a device over the wrong place, such as nerves, blood vessels, the front of the neck, a fresh injury, or a bruise, can cause harm.
